Honor or Memorial Gift

If you would like to make an honorary of memorial donation, please contactthe AFMO Office at 212-397-3700. An acknowledgment card will be sent to the individual letting them know of your generosity. Please include the following information in your email:

• Person you wish to honor or memorialize
• Name and address of the person(s) to send the acknowledgment card
• Donation amount ($36 minimum)
• Special instructions (if any)
